Preparing for Your First Appointment with Dr. Levin
Congratulations on taking the first step to find a partner in your search for optimal health. In the office of Dr. Warren M Levin, you will be treated as an individual, and your concerns will be viewed through a whole-body approach.
Your first visit
You will find as you enter our calm, intimate, and scent-free office a trained staff ready to provide you with timely, compassionate and individualized care.
Your first visit with Dr. Levin may be as long as 90 minutes. He will review your medical history and any prior lab work. He will listen to your concerns and answer your questions. More tests may be recommended at this time. Out-of-town patients may request the lab work to be done the same day.
Your second visit
After we receive the new lab results, we will schedule a follow-up appointment with a physical exam if needed. This visit generally lasts 90 minutes, depending on the information to be discussed. The visit is scribed and you may request a copy of the notes.
Test results are part of your medical record. You will receive a copy of your test results at no charge during your visit. These copies are your responsibility and should be added to your existing personal medical files. Please note that there will be a charge for additional copies including copies sent to another physician.
Treatment costs
Patients are treated as individuals. Therefore, treatment costs may vary from one patient to another. However, Dr. Levin and his staff will do their very best to estimate the cost of services that are needed as a result of your test results and treatment plan. Policies
* All patients are required to have a Primary Care Physician (PCP) – this practice does not admit patients to the hospital in case of a medical emergency.
* This is a fee-for-services based practice. Full payment is expected at time of service.
* We require 10 business days advance notice to cancel (or reschedule) your 1st appointment as a New Patient or your 2nd Long Consultation appointment with Dr. Levin. We request 3 business days advance notice for an Established Patient to change an appointment. We do not accept cancellations on Friday, Saturday or Sunday, since the office is closed on those days. You will be charged a cancellation or no-show fee for inadequate notice of appointment cancellation..We do not accept any cancellation or appointment change via e-mail.
* We have a no perfume or aftershave policy as some of our patients are highly chemically sensitive.
